Causes of and treatment for premature ejaculation

Premature ejaculation refers to ejaculating within a short time during sexual intercourse without being able to satisfy a partner. For example, just like how our heart rate is controlled by the autonomic nervous system, the penile cavernous nerves related to erection are also governed by the autonomic nervous system.

Many premature ejaculation patients have shown that the sensitivity of the glans is particularly higher compared to others. Dorsal penile nerve block is the most effective surgery for delaying ejaculation time by blocking the dorsal nerve, which is responsible for sensory nerves, thereby controlling the sensitivity of the glans.

For example, if a numbing cream mixed with anesthetics is applied to the glans and sexual activity is performed, a delay in ejaculation time can be observed. Based on this principle, dorsal penile nerve block is performed by cutting some of the nerves responsible for sensation in the glans to reduce its sensitivity. This surgery is effective in about 90% of patients and is carried out under local anesthesia, lasting approximately 20-30 minutes. After a recovery period of about two weeks, normal sexual intercourse becomes possible.

Premature ejaculation is most common in young men in their 20s and 30s, and the main cause is overly sensitive glans and penis. After the dorsal penile nerve block is performed to desensitize the area, dissolvable sutures are used, and within 1-2 weeks, the sutures dissolve.
